Case 45: Justice & Personal Responsibility (26 Apr) CLOSED

Respond collectively and thoughtfully to the questions at the end of Case 45, p. 718.

  1. 1. I personally believe that Jeffrey O the 71 year old should recieve the ICU bed.
    2. No my answer would not be different if the two ages were switched.

    My answer is based on the fact that the 22 year old who is younger does drugs and is in ICU because of that and the 71 year old is in the ICU for an automobile accident.

    3. This is a tougher question to answer. If they were both healthy and Jeffrey O was admitted first then I think that he should have the room based on the first come first serve being that their reasons are the same. However, I read an article about healthcare and giving it to the younger before the elder etc. and in relation to the two articles it would be assumed that the ICU room would go to the 22 year old regardless if Jeffrey got their first simply becuase he is already 71 and has lived his life and the 22 year old has more to live. (Not saying I agree, but that was the basis of the article and how this would apply)

  2. 1. I think that the 71 year old should receive the bed.
    2. No my answer would not change.
    Like michelle my answer is based on the fact that the 22 year old is in his current condition because of his own choices.
    3. If the two patients came to the ER with the same problem from a car accident and the 71 year old arrived first then the 71 year old should get the bed. Age has nothing to do with the type of care that people receive. Well at least in my opinion I feel that age should not determine what care you get.

  3. 1)I think that if both patients are equally critical then Jeffrey O should receive the bed since he was injured by accident and wasn't in control of his injuries. Donald R knew what the consequences could be from doing drugs, but he did them anyway.

    2)I wouldn't change my answer if their ages were switched because my answer to number 1 wasn't based on age.

    3)If Jeffrey was admitted first and they were both injured in an accident then I think Jeffrey should get the bed. My answer is solely based off of the fact that he arrived first and deserves to be treated equally regardless of age. I understand where age could be called into question, but he is still a human being, and I think it would be unethical to turn him away.

  4. 1.) I'm not sure that this judgment is supposed to be made in medicine. I'm sure that nurses from other floors could help in assisting either one of the patients. However, for the sake of the argument, I will say that Donald R. deserves the bed simply due to the fact that his condition needs to be addressed immediately and perhaps the care can lead to a life-change.
    2.) I do believe my decision would change if the ages were switched. I think I would rather save a 22 year old drug addict than a 71 year old drug addict, if given the choice.
    3.) If Jeffrey was admitted first, why was he not already admitted? I would give it to Jeffrey first. Hopefully if he has been the hospital for an hour, nurses know his exact care plan. Donald must be assessed first to see what they need. Being able to carry out a thought-out plan as opposed to a plan on a whimp in this case would have more authority. Age should not have a factor in your care path. There are arguments for both sides about why they deserve the care more.

  5. 1. I think that Jeffery O should recieve the bed due to the fact, that he is in good physical condition and more likely to recover to a good, healthy life, whereas Donald R could return to his drug use

    2. I would not change my answer because my answer was based on who would live a better more fulfilling life

    3. I think that age should not influence our decisions based on care however it most certainly does, and in healthcare, it most certainly is not first come first serve. Donald R in this situation is only 22 with a long life ahead of him, whereas Jeffery O is 71 the preference should be given to the youth

  6. 1. I believe Jeffery should receive the bed. He sustained his injuries by an force oustide his control. On the other hand, Donald's condition was done from his own hand.
    2. The answer would not be different.
    3. Arguments could be made from both sides here. One may say that the younger man should receive the bed simply because he has more of his life ahead of him. Another could say that the older man should recieve the bed, since his age makes him more likely to be in need of extra care. In this situation though, since Jeffery arrived at the hospital first and their conditions are equal, he should recieve the bed. Now if one car accident left one man in worse shape than the other, he should recieve the bed since he is in more critical condition. But if their injuries were similar, it should be first come first serve.

  7. 1) I also believe that Jeffery O deserves the bed in the ICU.
    2) No my answer would not be different. Jefferey O is there because of an accident, he otherwise has a healthy life. Donald is there because of a personal choice and he created the injury to himself.
    3)I believe that treatment should be based on a first come first serve basis. Age should not be a determinate in treatment.

  8. 1. I think that the older man should receive the bed because he did not do anything to his body that has landed him in the hospital. The drug addict however knows the consequences of his habits and I think he should not be given the ICU bed.
    2. I dont think the answer would be different based on ages of the individuals because I think that it is the drug use that makes me not want to give the young guy the bed.
    3. In this case I would say it would be whoever was in the hospital first because there is no other fair way to determine who should get the bed in the ICU

  9. The hospial should do everything in their power to treat both, or find another hospital willing to take the other patient. I would ultimately give the bed to Jeffrey O. this does not mean I would feel good about it. Certainly, Donald R. made his own decsion to use the drugs, but I do not feel he should be treated any different as a patient because of his choices. If it is the job of healt care providers to treat pateints, then one patient can not be more important than the other.
    My answer would not be different. Either way it would be difficult to turn away a patient no matter how old they are or because of the choices they have made.
    I would give the bed to the person who came in first. Age should not matter, but I do not think a person that is a drug addict should be turned away because they are a drug addict. Certainly, if one was a drug addict, was treated, released, and then back in a month later for drug related issues in the medical field, and by their standars he is a patient and should be treated as such.

    2. No my answer would not be different based on age.
    3. Jeffrey O should get the bed because he was admitted to the hospital first. Donald R should still recieve care elsewhere though. Hopefully someone is released from ICU soon so Donald R can be admitted.

  11. 1.) I believe that the "bed" status should be based on need. Those who are in poorer health and need attention immediately should be given the bed. If both need immediate attention then it should the individual that was at the hospital first should keep their position.
    2.) I do not believe that the hospital should change their opinion if the ages were switched. It is not the job of the hospital to judge individuals, it is to treat them.
    3.)Like stated before, care should be given on a need based system. If both individuals need the same amount of care then it should be distributed on the fisrt come first served basis.

  12. I agree with Mary, whichever patient arrived first should receive the bed, and no my opinion would not change if the ages were reversed. Whoever was admitted first, in this case it was Jeffrey O, should get the bed.

  13. Which ever patient arrived first should recieve the bed. If both patients arrived at the same time, then which ever patient is in the most critical state, with the greatest potential for survival should recieve the bed. If Jeffery O was a 71 year old man that was already chronically ill with little chance of surviving i don't think I could justify giving him the bed. But if it comes down to it and both patients are equal in the time that they were initally seen and in the severity of their condition, I suggest that we resort to using alphabetical order or maybe even "Rock Paper Scissors." It's unethical to ration health care based on a patients age and/or lifestyle choices. If a convicted murderer was your patient you would be obligated to treat him the same as any other patient you may have. We are all humans with intrinsic value and are entitled to the same health care.

  14. Whoever has the best chance of making a full recovery should be taken into the bed. Personally biased on the situation, I would take in the older man who seems to be a “good citizen.” If this could be a totally unbiased situation, the person with the best chance of a full recovery should be taken into the bed. No matter who is who and what they have done, an unbiased decision should be to take in the person with the best chance of full recovery.
